We have made it really easy for you to make your appointment go smoothly as possible. First, we have posted our Patient Information Form and Consent Forms here in PDF fillable format. These forms allow you to fill them out on your own computer in the convenience of your own home. After you fill out all the fields, then you need to print it out from your printer to bring to your first visit.
IMPORTANT: These forms do not allow you to save the data for legal reasons.
Also, you must sign this form in the presence of our office staff.
Also, please bring a list of your current medications. If you have medical or dental insurance, bring your card to your appointment. We require your insurance information 24 hours prior to your appointment. This will save time and allow us to help you process any claims.
Please arrive 15 minutes early to your first appointment.
IMPORTANT: A parent or legal guardian who has power of attorney to make medical decisions must accompany all patients under 18 to ALL appointments including cleanings and consultation visits.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, heart murmur, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, antibiotics for a heart murmur, etc.)
Current X-Rays
If another dentist has taken x-rays, you may request that they forward them to our office. However, we highly recommend that you pick them up yourself and bring them with you, as most of the time the x-rays do not arrive before your appointment; and, we will need to have them at your visit to give you a comprehensive exam. If additional films are necessary, they can be taken at our facility. If your bitewings are older than 6 months we will need to take new ones. If your panorex x-ray is older than 3 years we will need to take a new one.

Comprehensive Exam:
This appointment is recommended for any patient who is new to our practice so we can get a baseline of your needs. Whether it has been 3 months or 30 years since your last check up, we invite you to this relaxing appointment where we can discuss your dental concerns and options. You can expect the following services in this visit:
- Digital x-rays that will take pictures to diagnose areas of concern in the teeth, gums, and bone where our own eyes cannot see. Many times we can diagnose a disease state early or will detect a cavity early so you will not need drilling. have discomfort during your dental visit. These x-rays also will provide our staff with the best look you can get at your mouth tissue, teeth, and bones so we can detect conditions with more accuracy.
- Gum Measurements are taken by using a small probe instrument (there is no pain involved) and this soft tissue exam is a simple test to make sure of no gingivitis or gum disease and to measure gum recession lines. More and more health reports are coming out with direct correlations between pancreatic cancer and heart disease in people who have chronic periodontal disease (gum disease). In other words, patients who have chronic periodontal disease have much higher incidence of pancreatic cancer and cardiovascular disease.
- Comprehensive Exam takes into consideration periodontal findings, overall health and condition of the oral tissues, presence of tooth decay and infection of teeth, condition and stability of existing restorations, and the development of a treatment plan to resolve any problems the patient may have. Our dentists are concerned about and put a lot of emphasis on restoring the mouth to a healthy status. We routinely perform esthetic improvements of the mouth and produce excellent cosmetic dentistry results with our advanced technology, but it is up to you, the patient, if you have esthetic concerns, to mention them to the doctor in this visit. We don’t push cosmetic dentistry as other offices do, as your mouth is what you want it to be and if you have concerns to change it, we want you to voice what improvements you want us to help you with. Our philosophy is simple- that a healthy mouth is happy mouth. Again, if you have concerns about esthetics we will present all options for your concerns but cosmetic improvement is a personal choice that ultimately, you the patient, need to make, before we intervene. We will not convince you that you need something esthetic if you do not want it or do not need it.
- Complimentary Oral Cancer Screen This is a visual screen where all of the gums and tissues and the tongue are looked at to check for areas that can turn into potential pathology like cancer. It is SO important to find these areas early and just because you may not use tobacco, does not mean you cannot have cancerous tissues. Cancer screens are not done on a routine basis in many dental offices, but with the training our dentists have received in cancer treatment of the head and neck, they agree it is important to find areas of concern early rather than later. We have diagnosed many cases of pre cancerous lesions through this exam and have helped many patients detected cancer early. We have seen devastating outcomes of patients who have developed cancer in the mouth where diagnosis has been delayed or undetected for whatever reason. Therefore, we attempt to diagnose pre cancerous lesions as early as possible to prevent the misery patients have to go through if the lesion is advanced.

The second type of initial appointment we offer is a complimentary consultation with Dr. Novak This allows an opportunity for you to come in at no cost to you to ask questions about your dental concerns and to get an opinion of the dentist. This appointment is a relaxing appointment only involving talking between you and Dr. Novak. He has successfully treated so many people who are nervous or anxious or who are embarrassed about their smile, which has kept them away from a dental office; and, he has literally relieved thousands of their dental concerns. We encourage you to try this no pressure visit to meet our staff and talk to Dr. Novak as your case is probably not something that he has not seen or has been able to treat already, so there is little need for uneasiness.!
